Release of Liability:
I understand all reasonable safety precautions will be taken at all times by Holy Cross Parish and its employees and agents during the Catechesis of the Good Shepherd sessions and activities. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree to indemnify and hold harmless Holy Cross Parish, the Catholic Diocese of Madison, its leaders, employees and volunteer staff from any and all claims arising from or in connection with participating in the Catechesis of the Good Shepherd program.

Commitment Agreement:
I will do my best to arrive on time and communicate any absences to my child's catechist and the CGS coordinator. I understand that it is disruptive to the atrium environment for my child to be late, so I will find the atrium closed to late arrivals after 9:05 am unless previously arranged with my child's catechist. If I plan to stay during the optional playtime in the nursery with my other children not in CGS, I agree to follow the nursery guidelines (please read attachment below).
